An exciting opportunity has arisen for a Healthcare Navigator to join a public sector organisation based in East London.

The role will be working within the Housing Solutions department, playing a crucial role in the resettlement of rough sleepers. The role will be key to supporting vulnerable clients to recover from rough sleeping and achieve positive outcomes.

The most successful applicant will ideally have previous experience working within a health or social care environment, working in a community setting and supporting individuals with complex needs. Knowledge of local social and healthcare services and understanding of homelessness issues would be desirable.

Main duties will include

  • To offer early intervention and ongoing support to prevent individuals with multiple and complex needs from returning to rough sleeping following discharge from hospital

  • Support the health and wellbeing needs of the individual through activity such as: specialist healthcare service, specialist misuse programmes, counselling, welfare and debt advice services, etc.

  • Accompany individuals to appointments and help them to ensure an appropriate response whilst empowering them to plan their exit from the service and onto sustainable ways of living

  • To provide information of the best options and care available and where that may be

  • Involvement in assessments to help co-design a person centred, strength based support plan according to their needs

  • Provide continual support to ensure that the individual is resident in their accommodation, settled into their accommodation and they have access to and are engaging with primary and secondary care services to meet their healthcare needs

  • Adopt a multi-disciplinary approach with a focus on coordinating support. Make connections with a wide range of support services to facilitate support and assistances for individuals to address social and health issues and other personal barriers to sustaining housing
